  5.     DIED. Bill Graham, 60, rock-concert promoter who ushered the
  6. psychedelic era on-stage; in a helicopter crash near Vallejo,
  7. Calif., that also killed the pilot and another passenger. In the
  8. late 1960s Graham's Fillmore West auditorium in San Francisco
  9. and Fillmore East in New York were spawning grounds for the
  10. careers of the Grateful Dead, Jefferson Airplane, Janis Joplin,
  11. Santana and other counterculture stars.
